First of all just like when you lose your wallet, debit or credit card you inform the police station and file an FIR, losing your voter id also requires you to do that. So, visit your area police station and file for an FIR by explaining the police about how you lost it.
They will provide you a copy of FIR, keep it safe with you because you will need it to be attached with the duplicate ID request form.
Now visit Maharashtra CEO website here: https://ceo.maharashtra.gov.in/Defaultc.aspx and download the form 002 which is required to file you application of duplicate voter id card issuance. You can also visit nearby election commission office to get a printed copy of the form if you don’t have access to internet.
Fill in your details and ensure all the credentials are filled properly and there are no spelling mistakes or wrong information filled.
Now after filling in all the details, attach a copy of the FIR you received from the police station and along with it attach other identity proof documents such as Pan Card, Driving License or Gas Bill, Electricity Bill or Passport.
You can now put all the documents and form in an envelope and submit it to your nearest voter registration office. You will be provided an acknowledge receipt by the officials. Few week after you will be able to check the status of your duplicate voter id card.
